The Old Meeting House, a Unitarian Church (1702) known as 'Mill Walk Unitarian Church', 42 Stockhill Gate, Mansfield, Nottinghamshire, NG18 1LR. The memorial comprises two panels each with the dedication: 'In remembrance (names) Their name liveth for evermore.' and these are in the porch of the building. Replica panels have also been installed in the Old Meeting House gardens.
